Hospice in the Greater Central Valley

Community Hospice has provided compassionate care to terminally ill patients and their families in Stanislaus and South San Joaquin counties since 1979. As a nonprofit organization founded by community members committed to the concept of “death with dignity” we are firmly committed to the communities we serve. Our nonprofit promise to you is that we will provide care to anyone who needs us, regardless of ability to pay.

What does Community Hospice do?

Hospice is not a place, but a philosophy of care. The hospice concept emphasizes comfort care , rather than curative treatment. At Community Hospice, our approach to patients is holistic , which means that we view our patients as having unique physical, emotional, spiritual and social needs.

Our patient care teams develop a comprehensive plan of care that addresses a patient's physical, emotional and spiritual well being. Community Hospice regards the entire family, rather than the patient alone, as its "unit of care," and offers supportive services for all family members.

We can provide our services to patients in their private homes, in skilled nursing facilities and residential homes for the elderly, or at our inpatient facility, Alexander Cohen Hospice House , located in Hughson.

  Emotional Support, short-term counseling
  Referral to community counseling agencies
  Referral to appropriate community support agencies (Meals on Wheels, MSSP. etc,)

Volunteer Services are available and can be assigned to help meet patient/family needs:

  Practical caregiver support (i.e. errands)

Bereavement services and grief support for family members

  Support groups
  Telephone contact
  Supportive and educational mailers

Spiritual Services

  Spiritual Counseling
  Work with clergy of patient's choice
  Coordinate with church/synagogue for support

Therapies

  Physical, Occupational, Speech and/or dietary therapy/counseling
